Supported integrations

  1. FHIR R4 inbound

    Accept Patient, Encounter, Condition and Observation resources, singly or in bundles, mapped to members, encounters, conditions and SDOH observations. Every resource is logged, and new patients go to duplicate review instead of being merged automatically.

  2. FHIR R4 read

    Read-only access to Patient and Observation resources for connected systems.

  3. FHIR SDOH export

    A Gravity Project Z-code catalog and FHIR SDOH bundles for each member. Batches go to the destination your organization configures, or are exported for you to deliver.

  4. ADT event ingest

    Available

    Send admit, discharge, transfer and emergency-visit events to Relay's API. Admissions notify the care manager, discharges create follow-up tasks, and a dashboard shows alerts and feed health.

  5. Eligibility checks

    Check coverage through an Availity clearinghouse connection, or New York's MEVS gateway. Each result records whether it came from a live source, so coverage is never inferred.

  6. Community resource imports

    Load community resources from 211 (HSDS), Open Referral and FindHelp export files as versioned releases you can roll back.
